Prior to your first appointment, please fill out our intake paperwork, consent for treatment and our financial agreement. This will be sent to you electronically. Please contact the Office Coordinator at 206-925-3762 if you have difficulty completing the paperwork.
Please bring the following to your first appointment:
Identification and Insurance Card
Referral from your healthcare provider (if available)
Any copies of diagnostic tests (VNG, calorics, MRIs, etc.)
Medication list–please bring an updated copy of your medications, including prescriptions, over-the-counter medications, herbal supplements, and vitamins. Please include dosage, frequency and how long you have been taking this medication.
Appointments are usually scheduled for 55 min. Please arrive 20 minutes early for your first appointment if you have not filled out the necessary paperwork available above.
The Leschi business area has 2 public parking lots on each end of it. Both lots have 2 hour free parking spots available. Do not park in the rows labeled for “Moorage.” Additionally, there is free 1 hour parking on the street in front of our office. Finally, there are 4 visitor spots available in the building parking lot located behind the building. Please do not park in numbered spots as those are used by other businesses.

It is best to wear comfortable, flexible clothing to your visits. Please avoid high heels or flip-flops. It is important to discuss your normal footwear with your PT as part of your treatment plan.
During the first visit, your physical therapist will want to discuss your current symptoms and medical history in some detail.
For patients being seen for dizziness and related symptoms, a variety of tests will be performed to assess the function of your vestibular and nervous systems. Additional tests will be done to assess your balance and walking abilities. Your therapist has many years of experience working with patients who are dizzy or imbalanced. Be assured that the evaluation will be done in a way that is manageable for you to tolerate.
For patients being seen for orthopedic symptoms, your therapist will look at how your entire system works to create the movement patterns causing your problems. This will be a head-to-toe assessment of both orthopedic and neurologic factors to find the key drivers to your problem.
Cascade Dizziness & Balance PT is contracted with Original Medicare, Premera, Regence, First Choice Health, Kaiser and others. Because of the ever-changing health insurance environment, we strongly advise you to call us and your insurance company to verify your benefits.
Below are insurances that we are NOT contracted with and, therefore, unable to bill.
- Most Aetna plans
- All Cigna plans
- All HMO plans
- All Humana plans
- Physicians Care Network (Polyclinic) plans
- All UnitedHealthcare plans
- Medicaid plans (Apple Health/Molina/Community Health of WA, ProviderOne). We are unable to see Medicaid beneficiaries even as self-pay patients.
If your insurance company is not contracted with us, we offer the following cash rates:
- New Evaluation $175
- 1 Hour Appointment $160
- 30 min Appointment $120
We are able to provide a Superbill for you to submit directly to your insurance company for reimbursement.

A Medicare Advantage plan replaces your Medicare coverage. Please call ahead to inquire if we take your plan.
