Prior to your first appointment, please fill out our intake paperwork.  If you are being seen for an orthopedic problem, please fill out only pages 1-5 of the intake paperwork.

Please bring the following to your first appointment:

  • Identification and Insurance Card
  • Referral from your healthcare provider
  • Any copies of diagnostic tests (VNG, calorics, MRIs, etc.)
  • Medication list

Appointments are usually scheduled for 55 min. Please arrive 20 minutes early for your first appointment if you have not filled out the necessary paperwork available above.

During the first visit, your physical therapist will want to discuss your current symptoms and medical history in some detail. Then a variety of tests will be performed to assess the function of your vestibular and nervous systems. Additional tests will be done to assess your balance and walking abilities. Your therapist has many years of experience working with patients who are dizzy or imbalanced. Be assured that the evaluation will be done in a way that is manageable for you to tolerate.

It is likely that your physical therapist will use video goggles to assess the function of your vestibular system. The results of these tests can be recorded on DVD to review with you. You are free to take this DVD home to share with family and friends if you like.

 

If you have Medicare, please bring an updated copy of your medications, including prescriptions, over-the-counter medications, herbal supplements, and vitamins. Please include dosage, frequency and how long you have been taking this medication.  Thank you for your cooperation in this matter.

NOTE: Due to there being several changes with insurance in 2014, those insurance companies that we are in- network providers for may have supplement plans that we are out-of-network with. Therefore, it is your responsibility to call your insurance company to verify your benefits for physical therapy services.

For patients with Aetna or United Health Care, we are not in-network providers for these organizations.  Please confirm your out-of-network benefits before arriving for your first appointment.

For patients with Regence, Regence now requires pre-authorization for physical therapy services.

If your insurance is not contracted with Cascade Dizziness and Balance PT or our services are not covered, we offer a 25% discount for cash payments on the day of service.

There is ample FREE parking available around the clinic.  1 hour parking spots are along the street in front of our office.  There is a 2 hour parking lot to the south and north of our office.  For people with mobility challenges (wheelchairs, walkers) there are visitors spots available behind our building.
